Formal Complaint Form


Please use this form to submit a formal complaint. You must complete all applicable sections of this form before the complaint will be reviewed. Provide the details that your support your complaint. Give a description of the steps that were taken to exhaust the institution's informal complaint process. Include any additional information that pertains to your complaint.

Background Information

Whenever possible, we encourage that incidents are resolved directly with the staff or department involved. Often a complaint can be resolved in this way. However, if an informal approach is neither successful or advisable, the Formal Complaint Form should be submitted.
