Report Bias Form


This form is intended to be used to report bias related incidents where a student has been harmed or caused the harm.

Incidents reported here will be addressed through the process as outlined in the Community Standards under 7. Bias Related Behaviors Process. Incidents where an employee caused the harm will follow the University's Harassment and Discrimination Policy.
