This form is only monitored during regular business hours (Monday-Friday 8:30 am - 4:45 pm) and may not be immediately reviewed. Contact University Police (504-865-3434) IMMEDIATELY if there is a risk to self or others.
This form is used to report information necessary for the University to address alleged discrimination, harassment, sexual or gender harassment, domestic or dating violence, stalking or retaliation pursuant to the University's Title IX Policy.
Title IX Mandatory Reporters - Reporting Obligation
All University employees, other than counselors, pastors, or advocates and those employees legally regarded as confidential sources must bring reports of violations of this policy, including sexual and gender-based harassment, assault, and violence that they observe or learn about, immediately to the attention of the Title IX Coordinator or appropriate Deputy Title IX Coordinator. Failure to do so can result in discipline, up to immediate discharge. The employee must report all relevant details about the alleged policy violation, including the name of the alleged victim, the accused, any witnesses, and any other relevant facts, including the date, time, and specific location of the incident. Students who are not also employees do not have reporting obligations under this Policy. Nevertheless, students who have witnessed relationship violence or sexual misconduct are strongly encouraged to promptly report such information to the University.
- If you are classified as a confidential source, you do not disclose any information to the University.
- If you are classified as a mandatory reporter, you must report all relevant information that has been reported to you, including the names of the respondent and/or complainant, dates, locations, etc.
Campus Security Authority (CSA) Reporting Guidelines
Campus Security Authorities carry the responsible reporter responsibility in regards to Title IX reporting, but they must always report aggregate data to University Police. Please note Sexual Harassment is not considered a Clery reportable offense; Clery offenses include sexual assault, stalking, dating violence, and domestic abuse. You have two options for reporting under your CSA obligations:
- Complete the Crime Statistic Report Form and submit it in-person to LUPD.
- Complete this incident reporting form and select Title IX and CSA in the Nature of this Report drop down in the next section.
