ACADEMIC SANCTION FOR CLASS: Faculty determine when a student is responsible for Academic Misconduct and the Dean of Students office meets with the student for an accountability-based intervention, often resulting in institutional resolutions/outcomes (ie-written warning, education resolutions, probation, suspension etc.) which are in addition to any grade consequence determined by the faculty member. Institutional outcomes do not limit the individual faculty member's academic freedom and the right to maintain academic integrity within the learning environment. This field is required.

PERMANENT F OUTCOME: If you are issuing Permanent F please select your justification. If you are not assigning a Permanent F, please skip this question. A faculty member may issue a grade of a permanent "F" as a result of academic misconduct. For additional information go to the Student Handbook and consult the Student Code of Conduct section.
