Student Concerns (SOS) / Code of Conduct Reporting Form


Students, faculty, staff, and other members of the campus community are encouraged to report student concerns and/or situations that may be in conflict with established standards of behavior as outlined in the College Code of Conduct (which can be found on the NMC website), if members of the campus community are exhibiting concerning behavior that may result in harmful, threatening, or disruptive conduct, or if you have a concern or complaint about something at the College.

ADDITIONALLY, Campus Security Authorities (CSAs) may also use this Reporting Form to report crimes by utilizing the “Campus Security Authority Report” in the drop down menu below. However, this is not an emergency service. For emergencies, call 911.

By submitting this report, you are stating that all information contained in the report is true and correct to the best of your knowledge. Furthermore, please realize that the individuals involved may be afforded certain privacy rights under the Family Educational Rights and Privacy Act (FERPA) which establishes requirements regarding the privacy of student records.

Once a report has been received, the appropriate NMC staff will review the report to determine an appropriate course of action.
