If you have just received an academic integrity incident report, the Academic Integrity Office recommends that you write down your description of events in a Word document as this form will time out. We recommend waiting 24 hours to think about your statement and get your documentation together before submitting this form.
After completing and submitting this appeal form, call the departmental office of the class to schedule an appointment with the Department Head to review your appeal. You will need to meet with the department head within 15 days of receiving the notice of an academic integrity incident report. If you need more time to meet, please email AcademicIntegrity@missouristate.edu with details of your request for an extension.
