The chapter listed below hereby notifies the Office of the Dean of Students of its intention to conduct a social event.
Chapter events with alcohol require this Function Notification Form to be submitted no less than six (6) business days prior to the event. The Office of the Dean of Students reserves the right to not approve a late request and/or a poorly planned function. Failure to comply with chapter safety plan may result in sanctions or loss of future events. Notification for other events that do not meet the registration requirements can be submitted at anytime prior to the event.

The Officer in Charge is the lead member of the Chapter Safety & Risk Team for the event. The members of the Chapter Safety & Risk Team agree to remain alcohol and substance free for the event, have been trained by DOS and MSU VOICE Center annually, wear identifying t-shirts throughout the event, control who enters the event and check identification of all guests.
