This form is for reporting incidents that may be violations of the University’s Discrimination, Harassment and Retaliation Policy and the Montana Board of Regents Title IX Policy. This includes incidents of protected class discrimination and harassment, including allegations of sexual assault, domestic and dating violence, stalking and retaliation. The Office of Institutional Equity will receive this report.

It is important to know that all MSU Employees are required to report known or suspected discrimination or harassment based on an actual or perceived protected class. All employees--except only those with a legal privilege, such as medical providers, mental health counselors, and VOICE Center advocates--are subject to the mandatory reporting obligation.
