If you have been suspended for disciplinary reasons, you may submit a written petition requesting that the notation of that suspension be removed from your transcript. Your petition should be attached to this form as a separate PDF document.
More information about the petition for requesting that a notation be removed from a transcript can be viewed in Section 8 of the Student Discipline Procedures at Student Discipline Procedures.
At minimum, the petition should include:
• A description of activities/involvement since the suspension was imposed (education, employment, service hours, etc.)
• A review of the sanctions that have been completed as a result of the suspension
• A reflection about the impact of the suspension
• A description of why the notation is being sought at this time
