This form can be utilized by any member of the campus community wishing to file a report on their own or anothers' behalf after a possible violation of our Title IX regulation, concerning gender discrimination or sexual misconduct. This form is received by Alisha Carter Harris.

If you are experiencing an emergency, or if there is an imminent risk to your well-being or that of another person, contact NCF Campus Police at 941-487-4210. If you are off-campus, call 911.

**Information submitted through this form outside normal business hours may not be reviewed until the next business day.**

This form does NOT automatically initiate a Student Conduct Process. This form provides you with an opportunity to inform the Title IX Coordinator of your experience and to receive advice and support. Should you wish to file a complaint against a current New College community member, you may choose to use the report generated by this form as your "Complaint Form" or you may choose to augment your Complaint Form with additional information in the future.
