Please use this form to refer students for alleged student code of conduct violations, academic misconduct, or to report a concern to the Care Team (formerly the Behavioral Intervention Team/BIT).
Please note, reports are processed during college business hours between 8:00 am and 4:30pm. They are not reviewed after business hours, weekends or holidays. If you have a threatening situation requiring immediate attention, please call Normandale’s Public Safety Office at 952-358-8280 or 555 from a campus phone.
- You will receive an electronic confirmation the report has been submitted to the Dean of Students Office. You will also receive a follow-up email or call from the Dean of Students or the hearing officer to confirm receipt.
- The Dean of Students will review the report and develop an action plan. The more detailed information you are able to provide in the section titled “Description of Incident,” the better we are able to respond. In most cases, the plan will include a formal investigation into the situation. The Dean, or a representative, will meet with the student of concern and any potential witnesses indicated in the report.
- After the investigation, appropriate sanctions may be taken, up to and including suspension from the college.
- Other students may be referred to the Care Team. This team is charged with monitoring students who may not have a conduct charge, but have been deemed a potential risk to themselves or to others. This team includes the Dean of Students, the campus psychologist, public safety, an academic dean, the Director of Student Life, and the Title IX coordinator. All members of this team have attended formal training through the National Behavioral Intervention Team Association (NABITA).
- When possible, you will be notified of the outcome of any investigation or conduct hearing. In many cases, the information in the final report is protected by the Federal Educational Right to Privacy Act (FERPA). As a result, you may not receive a detailed update regarding the outcome of a disciplinary meeting or mental health assessment. If the student is being removed from a faculty member’s class, the faculty member will be notified.
We thank you for taking the time to notify us of any conduct issues and/or concerning behavior. The information you have provided, including the identity of the student of concern, will be kept confidential whenever possible. Information that needs to be shared for the purpose of the investigation and resolving the concern will be shared with the appropriate people.
If you have questions about this form or the process, please contact the Dean of Students at 952-358-8926, DOS@normandale.edu, or stop by C 1050.
