Student Conduct Reporting Form


This form is for reporting violations of the Student Code of Conduct and/or any violations of the policies in the Residential Life Handbook. Examples may include drugs, alcohol (including amnesty), harassment, disorderly conduct, etc. Residential Life violations include quiet hours, escort violations, cohabitation, etc. If you question under which category a violation falls, please contact the Director of Student Conduct and Development at 660-562-1554. If you are a staff member of Residential Life, please contact your Professional Staff on Duty.

If this is an emergency situation, please contact University Police at 660-562-1254 or call 911 immediately.
