This form is for sharing concerns you have regarding the well-being of others on campus or concerning behavior that may not be considered a conduct violation but is worrisome nonetheless.

By sharing your concerns, you are helping to maintain Northwest Missouri State as a healthy and safe campus environment. Once received, the information will be reviewed and you may be contacted for additional information. If you wish to make an anonymous report of concerning or worrisome behavior, the report will be evaluated and a determination will be made for what follow up (if any) is necessary to address the concerns.
