Community Concerns Report


This Community Concerns Report may be used to share information with the Dean of Students Office about behavior, issues, or concerns regarding Northwestern University students. Expressing concern shortly after an issue arises or an incident occurs can facilitate early intervention and promote student success.

When completing this form, please be as thorough, specific, and objective as possible. While you are not required to provide your name, we have found that interventions are more effective when the student understands the origin and genesis of the report, including the name of the person sharing the concern. Additionally, because we may also need to contact you for further information, it is very important that you provide us with an email address where you can be reached, even should you choose to not provide your name.


Community Concerns reports are reviewed during regular business hours. The Dean of Students Office will acknowledge receipt of your concern when it is received so, again, please enter an email where you can be reached.


 

  • If this is an EMERGENCY or if a student is in IMMINENT DANGER of harming themselves or others, CALL 911 IMMEDIATELY or contact the Northwestern Police Department at 847-491-3456. Do not use this form to report events that present an immediate threat to health or safety.
  • On-call campus medical and psychological professionals are available for consultation 24-hours a day by calling 847-491-8100 (Student Health) or 847-491-2151 (Counseling and Psychological Services).

  • There is also a Dean on Call available for consultation after hours by calling 847-467-3022 (During regular business hours, simply contact the Dean of Students Office at 847-491-8430.)

Student of Concern & Other Parties

Please list the individuals (or organizations) involved and provide as much information in the listed fields as you can. It is especially helpful if you can include the student's NUID number (but it is OK if you don't). Witnesses and other people who might have information about the student or the situation should also be identified here.

If you don't know the name of a student or individual involved, please use the following format in the name field: FirstNameLastName (e.g., "JohnSmith"). Then describe them as much as possible in your description of events below. This could include the person's age, sex, physical appearance/characteristics (height, weight, skin tone, hair color/texture, eye color, facial hair, etc.) as well as a description of their clothing, accessories, and any distinguishing features or characteristics. You should also identify any known affiliations (school they attend, organization they might belong to, etc.).

When selecting the “Role” for each individual you include below, please refer to the following definitions:

  •  Subject of Concern: a person or organization who is the subject of the report, whether for a wellness concern, or because they are alleged to have violated University policy.
  • Complainant/Impacted Party: an individual impacted by the actions of another person or organization, particularly if those actions are alleged to have violated University policy.
  • Person with Information: an individual who has information about the reported situation, or the persons involved in it, whether or not they directly observed the situation.
