Hazing Incident Report


As always, our first concern is the safety and well-being of our students. If you or anyone you know feels unsafe or is being harmed, please call 911 or Columbus Police at 614-645-4545 or OSU Police at 614-292-2121.

This report is not monitored outside of normal business hours or on university holidays.

If anyone is experiencing harm or is in imminent danger, please call 9-1-1.  

 

Reporter's Information

Please provide your name and contact information. While you may submit this form anonymously, the university's ability to take action in response to anonymous reports is limited. Please consider adding contact information. Without contact information, we are unable to provide you with a response. If you want to remain anonymous without providing contact information, we encourage you to use the anonymous reporting option offered by the Office of University Compliance and Integrity. That system allows users – without disclosing their name or other identifiable information – to follow up on a report; to read the university’s response, or to provide additional information. Anonymous reports can be made using EthicsPoint. 

The Code of Student Conduct requires students to prevent, intervene or report hazing. The obligation to report is NOT fulfilled with an anonymous report.

You may have additional reporting obligations under Ohio law.  Specifically, Collin's Law: The Ohio Anti-Hazing Act Sec. 2903.311 (B) states:

No administrator, employee, faculty member, teacher, consultant, alumnus, or volunteer of any organization, including any primary, secondary, or post-secondary school or any other public or private educational institution, who is acting in an official and professional capacity shall recklessly fail to immediately report the knowledge of hazing to a law enforcement agency in the county in which the victim of hazing resides or in which the hazing is occurring or has occurred.
