This form is used to report academic concerns that do NOT involve discrimination, harassment, or other code of conduct violations. Use this form to report academic concerns involving instructor behavior, class policies, grades, unfair academic expectations or demands, etc. Students should attempt to resolve their concern(s) with their instructor prior to submitting this form. If the attempt to resolve is not possible or fails, submitting this form will begin the Academic Concern process. Academic Concerns submitted here are directed to the PPSC Academic Concern Manager, and then routed to the appropriate PPSC Instructional Division.
CCCOnline Classes - if your concern is about a CCCOnline class that you have taken or are currently taking, and you want to appeal a grade, have experienced misconduct by a student or instructor, or wish to report any other academic concern(s), you should, click here to report those concerns.
