Student Complaint Report Form

The Student Complaint Procedure provides a system to handle student complaints concerning allegations of discrimination, sexual misconduct, and other alleged violations of student rights. This form is completed by the student, and upon submission, is sent to the Office of Student Support, after Step 1 and Step 2 of the Student Complaint Procedure has been met.
