This is the preliminary form to initiate a Rock Valley College
student complaint regarding staff, faculty, another student, or academic class concern.
Compliments and accolades are welcomed below as well.
Complaint Types: Information Only, Informal Complaint, Formal Complaint.
Complaint Categories: General, Academic, Disability/ADA, Title IX Sexual Assault/ Harassment, Other Discrimination.

Please note that certain complaints may warrant the Dean of Students Office to take action regardless of your request.
Complaints Defined:
• Capricious final grade A Capricious Final Grade is determined by specific criteria, please see the capricious final grade policy link above to see if your complaint meets this criteria.
• A Formal Complaint is a concern that you would like to pursue beyond the informal channel and requires; a) meeting with the Dean or designee b) additional documentation and c) may require additional meetings and staff contacts.
If you are seeking a formal complaint, it will automatically require you to schedule an appointment with the Dean of Students Office to initiate the process.
• An Informal Complaint is the ideal resolution option. You and the individual would attempt to resolve the concern together first and we strongly encourage you to try to resolve the situation at this level. If you are unable to resolve with the person of conflict, you may still try to resolve at information level via the Dean of Students Office, Academic Division or respective department. If you are able to resolve the concern at this level, it generally does not require further information or action. (e.g. general concern, customer service, classroom, other student). The concern and resolution would be documented by the responding office.
• An Information Only Complaint is your submission of a preliminary student complaint form. You are not seeking any follow up or action- you "just want someone to know."
* Also note: While we encourage informal resolution for complaints, you may also pursue a formal complaint without going through the informal process first.
