All students interested in filing an appeal must complete this form. Please note that you will need your case number when submitting this request. Your case number can be found on your outcome letter, which was delivered to you via email, detailing the decision made in your case.

The appeal process is strictly a written process. You will not be granted an in-person meeting with the Appellate Officer to further discuss this case. Please provide all relevant information below or within the supporting documentation section.
