Bias Act Reporting Form


Your confidentiality and trust is very important to us. We will make every effort to respect your privacy. Please be aware, however, in certain circumstances involving safety or criminal action, we may not be able to guarantee complete anonymity.  If you want to make an anonymous report, you may use Silent Witness Form. A silent witness report will not initiate an investigation nor will you receive information in response.


Anyone reporting an incident through this form should provide either a phone number or an e-mail address so that we may contact you.

If this is an EMERGENCY situation, CALL 911 or contact the University Police Department at 607-436-3550. Do not use this form to report events that present an immediate threat to health or safety.
