Sexual Misconduct Report Form


This form should be used to report concerns pertaining to possible violations of Saint Peter's University's Sexual Misconduct Policy such as gender based discrimination, dating violence, domestic violence, hostile environment, sexual assault, sexual exploitation, sexual harassment, stalking, retaliation, or other forms of sexual misconduct. Saint Peter's University respects the sensitivity of the information that may be included in this form. Filing this incident report only notifies a limited number of University officials: Saint Peter's University's Title IX Coordinator, Elena Serra, and Title IX Deputy Coordinator, Anthony Skevakis. Please note that as mandated by Title IX as a federal law, the information included in this form is not confidential, but will be kept as private as possible in all efforts to protect individuals and the greater Saint Peter's community.

Completing this report does not substitute filing a report with Campus Safety or Police. To file a report with Campus Safety, please call 201-761-7400 and ask to speak with a supervisor or go to Campus Safety which is located on the first floor of Saint Peter Hall. Campus Safety can assist in privately contacting local Police.
