The University is committed to addressing issues of sexual harassment and sex discrimination. We need and appreciate you sharing critical incidents or concerns with us. Please review theTitle IX/Sex Discrimination policy online at [THIS LINK]. Where necessary, please see other related policies at this [LINK]. If you are looking for the Anti-Discrimination Reporting Form for other forms of discrimination that are not under Title IX, then [CLICK HERE].

This form is designed to receive information pertaining to any specific incident or concern of sex discrimination/misconduct and sexual harassment under the University's Sex Discrimination and Title IX Policy. Submission of this form does NOT constitute the submission of a formal complaint. Please reach out to the Title IX Coordinator to submit a formal complaint.

All reports of this nature are initally directed to the University's Title IX Coordinator, Dr. Sylvia Gray and her staff. Reports may also be directed to the Dean of Students office for undergraduate students, Human Resources for staff, the Dean or Director where applicable for graduate student programs or faculty, and the Provost's Office for other participants in the University's programs or activities. All reports are copied to the Sewanee Police Department but may not serve as official police reports. To file a police report, please connect with the Sewanee Police Department directly. While you may report anonymously by leaving out details of this report, a decision to remain anonymous may greatly limit the University’s ability to stop the alleged conduct, collect evidence, or take action against parties accused of violating the University's Policies.

Questions

Note that some questions are required and others are optional.
Identifying any involved individual as a complainant will result in the Title IX Office reaching out to provide support and where applicable and could also result in an interview of any person(s) named or involved in the incident. It is the University's responsibility to reasonably gather information regarding the incident. The person(s) named as the complainant (i.e. person making the direct complaint) as well as other involved parties may choose whether or not they wish to respond to our efforts to gather more information. If this report results in a formal complaint (a separate step where the complainant requests that the report be elevated to an investigation), it is the University's responsibility to alert the respondent to the allegation. The University must also presume that the respondent is not responsible for the alleged conduct and that determination regarding responsibility is made at the conclusion of the grievance process. Both the complainant and respondent have a right to file a report or formal complaint with the Title IX Office (see the Policy for more details). It is also required that the individual(s) be notified that retaliation of any type is strictly prohibited. Retaliation can be broadly defined, but includes intimidation, threats, harassment, and other adverse action threatened or taken against any reporter or other persons participating in the grievance and conduct processes. Retaliation should generally be reported promptly to the Title IX Coordinator and can also be reported to the Dean of Students Office for undergraduate students, Human Resources for staff, the appropriate Dean or Director for graduate student programs or faculty, and the Provost for other participants.
