Reports submitted under this form are routed to the Office of Equity and Equal Opportunity. Note that some reports may fall under the Title IX and Sex Discrimination Policy [Click here for the Title IX/Sex Discrimination reporting form and policy.]. This form also applies to incidents related to Title VI discrimination. Title VI discrimination is specific to incidents of race, color, or national origin in the university's programs or activities.
All reports of retaliation, harassment, and discrimination that are based on one or more protected classes are directed to the Senior Director of Equity and Equal Opportunity, and copied to the Dean of Students' Office for undergraduate students, Human Resources for staff, the Dean or Director where applicable for graduate student programs or faculty, the University's Title IX and Title VI Coordinators, and the Sewanee Police Department.
If for any reason you are unsure of if the information that you have should fall within this report, then you are encouraged to still submit a report under this form so that the Senior Director of EEO can review the report to assist in determining where the report should live. Mandated reporting is only for incidents of sex discrimination; it does not apply to other forms of discrimiation.
