Academic Integrity Code Violation Report Form


Alleged violations of the academic integrity code can be handled by the Faculty Reporter with or without consultation with the Academic Integrity Representative.

The Accused Student has the right to respond to the charges and the evidence (2 business days to respond). The Accused Student has the opportunity to accept the decision/sanctions or appeal the decision of the Faculty Reporter to the Academic Integrity Representative. The Accused Student has 2 business days to decide to accept the decision/sanctions or appeal. For a first time offense, the decision/sanctions of the faculty reporter are final, unless the student decides to appeal the decision.

Complete, accurate documentation is essential. Please complete the form below within 30 days of the alleged violation. You will be required to attach appropriate documentation at the bottom of this form. Please use objective, concise language as the accused student will receive a copy of this report.

The student was informed that the sanctions will be imposed after a check for prior violations is completed. If the Accused Student has been found guilty of a previous academic integrity code violation, the case must go directly to the Academic Integrity Hearing Board. The grade will be posted as Incomplete (if appropriate) until the outcome of the Academic Integrity Hearing Board. Sanctions imposed by the academic integrity hearing board may be similar to or more severe than those proposed by the Faculty Reporter and/or Academic Integrity Representative.(Required)
