Student Care Team Referral


Use this form to report a student’s conduct showing signs of distress that may be harmful to self or others and should be evaluated by the Student Care Team. Referrals are also appreciated for students who may need to be away from campus for an extended period or experiencing significant difficulties. Expressing concerns or providing updates in the most timely manner possible will greatly assist with early intervention and student success. When completing this form, please be as thorough, specific, and objective as possible.

If you believe the student is in IMMEDIATE danger of harm to self or others, call the Department of Public Safety at 517-264-7800. Submissions through the reporting form are not constantly monitored; therefore, the reporting form is never to be used for emergencies.
