Title IX Reporting and Concern Form


This form should be used to report incidents or concerns related to sexual misconduct and gender or sex-based harassment. This report goes directly to the Title IX Coordinator who will respond promptly, typically within 24 to 48 hours when the College is open. Once the report is received, the Title IX Coordinator (or designee) will reach out to the reporting party to follow up on the report and offer support, resources, and resolution options.

Submitting a Title IX Reporting and Concern Form does not immediately start emergency/crisis response or the sexual misconduct complaint process. However, based on the information you share, the College may take some immediate action to ensure that our community is safe.

Background Information

All faculty, staff, and student employees (i.e. RAs, SAMs, and SWATeam members) who are responsible employees are required to include their name and contact information. Others may submit this form without names or contact information, however without this information we will be limited in what we can do to follow up on the report.
