The reporting forms below provide community members with options to share information with various campus offices working to support student success and the health and safety of our community. This is an internal reporting system and is not monitored 24/7. 

Below is a list of the forms available in the dropdown menu and the subject of each form:

  • Refer to Student Conduct: To refer academic misconduct and other misconduct by a student, not including sexual misconduct.
  • Civil Rights & Title IX Reporting: Use this form to report a violation of an individual's civil rights (e.g. sexual misconduct and other types of discrimination).
  • BIT Referral: Do you have a concern about an individual and it isn't an issue of misconduct? Use this form to share a concern. Using this form does not necessarily result in a student going through the student conduct process. Examples of concerns include:
    • Written material containing alarming wording,
    • Statements indicating threats of harm to self or others,
    • Disregard for personal boundaries,
    • A significant change in behavior,
    • Erratic behavior,
    • Concerns of food insecurity and/or homelessness, etc.
  • Formal Complaint of Sexual Misconduct: Use this form if you wish to file a formal complaint and you are the receiver of sexual misconduct.This is submitting a formal Title IX complaint of sexual misconduct.
  • Appeal a Parking Citation: This form should be used to appeal a parking citation or the decision of the University Parking Appeals Panel
  • Campus Recreation Fee Waiver: This form should be used to request an exemption from the campus recreation fee.
  • Formal Student Complaint: This form is reserved for students ONLY to submit complaints pursuant to the Student Complaint Procedure.
  • Training Exemption Request: If you wish to be exempt from the Sexual Assault Prevention Training required of all incoming students during their first semester/term. Please visit the FAQs for more: Sexual Assault Prevention Training FAQs in Canvas 
  • Documentation Submission: Do you have documentation to submit for pregnancy accommodation, missed classes, or for a student conduct sanction? This form is a safe place through which you can submit documentation to be reviewed.

Early Alert Referral Program - Warrior Center for Student Success, Equity and Inclusion: Excessive absences, poor exam marks, a lack of participation, a low mid-term average, and other factors may lead one to submit an "early alert referral." Report Fraud, Waste, or Abuse: Anyone can report issues involving fraud, waste, or abuse outlined in TAMUS 07.01 and 10.02. Report Risk, Fraud or Abuse.

All offices receiving a form outlined in the above list communicate with one another and share information through the submission system. If you are submitting a BIT referral, the referral may also result in an "early alert referral" without requiring you to submit an additional form.

This form is for appealing a recently received parking violation. This form is reviewed by the University's Parking Services prior to being forwarded to the Parking Appeals Panel.
Community members are encouraged to share when they receive information that causes concern for a student. This is not associated with misconduct or violations of university policy or procedure. Referring matters through this form will not initiate the student conduct process.
You should submit this referral if you are in receipt of information about a potential violation of an individual's civil rights or Title IX protected rights. You should also submit this form if you are the recipient of behavior violating your civil rights and the behavior is not covered by Title IX.
We are required by law to have a signed formal complaint before we may start the 08.01.01 investigation and response process for Title IX. If you are the receiver of sexual misconduct and want to file a formal complaint, use this form.
This form is for alleged violations of the Code of Student Conduct that are not related to sex or gender discrimination. These reports are reviewed by the Student Conduct Office.
Students wishing to request accommodations supporting pregnant & parenting students should submit a request through this form.
