Academic Conduct Incident Form

Professors should use this incident form to report all potential academic conduct violations in their classrooms. Do NOT use this form to report non-academic concerns. Faculty members may submit a non-academic concern hereIf you have any questions about this academic incident form, please contact the Office of Student Conduct at scce@tamiu.edu or call 956.326.2265.

 

To ensure a comprehensive review, please consider submitting the following documentation:

  • Turnitin Report (if applicable)
  • Course Syllabus, particularly sections that involve descriptions, definitions, examples, and/or penalties for academic dishonesty.
  • Assignment in question
  • Video (proctoring videos, if applicable)
  • Assignment/project instructions
  • Other projects or information associated with allegations.

 

Please note the Honor Council may reach out to professors, to inquire about additional information or to participate in hearings should the student exercise their right to appeal.

Student(s) Involved

Respondent - means any student against whom a complaint or charge under the Student Code of Conduct is made. 

Reporter/Complainant -means any person who submits information indicating that a student may have violated the Student Code of Conduct. Information brought forth by the complainant may result in an investigation.

Witness - means any individual who may have information relating to a complaint.
