This form is for anyone to report concerns about a Tufts student who may be in need of additional support. This may include mental or physical health challenges, family concerns, food/housing insecurity, concerning behavior, or other personal difficulties. The Student Support team will review the form during normal business hours (M-F, 9a-5p), use the information you provide to coordinate a response, and reach out to help students connect to the appropriate resources and supports. Student Support may also reach back out to you for additional information.

Please note: this form is NOT for reporting emergency concerns. If you need emergency assistance, please call the emergency line for the Tufts University Police Department: 617-627-6911. For after-hours health concerns, you can also reach the Counselor on Call for mental health concerns (617-627-3360) or the after-hours nurse triage for physical health concerns (617-627-3350).
