Student Assistance and Support Services - Absence Verification Request Form


Due to the heavy volume of requests, please note that a student's individual request can take two weeks for processing!

The Office of Student Assistance and Support Services can provide action regarding requests for verifying absences due to documented circumstances, including medical emergencies, personal/family emergencies, military orders, or court orders. Please make sure you have reviewed the website for more detailed information regarding the specific circumstances the office will verify. Submitting this form will initiate the absence verification process. If you have specific questions regarding absence verification, please contact the Office of Student Assistance and Support Services at 704-687-0289 during regular business hours (Monday through Friday, 8:00am to 5:00pm). If you have an emergency or a matter requiring immediate attention, please contact UNC Charlotte Police and Public Safety at 704-687-2200.

Final Approval of all absences, missed work, and/or extended academic deadlines is determined by the instructor.

The Office of Student Assistance and Support Services reserves the right to grant or reject absence verification requests at any time.

Due to the high volume of requests received during the semester, students must submit their absence verification request form AND official supporting documentation by the posted deadlines for the relevant term. Students are responsible for reviewing the posted deadlines, which are published on the SASS website at the beginning of each term.

Your Absence Verification request WILL NOT be processed without original documentation provided to the Office of Student Assistance and Support Services. You can attach documentation to this request form. You may also fax, email, hand deliver, or mail original copies to the Office of Student Assistance and Support Services for formal verification.

This form should be completed by current students, however submissions not submitted by current students will be reviewed for consideration based on extenuating circumstances. Please note that unexpected circumstances do occur and can cause a delay in review, but every effort is made to process all requests as efficiently as possible.

Provide SPECIFIC information to support your request. If information is lacking, your request may be returned to you for further clarification, or not processed due to lack of specific information. Please adhere to posted semester deadlines, as outlined on our website. Requests submitted after the deadline will not be considered.

Information for COVID-19 Positive Students (READ FULLY): Beginning with the Spring 2023 semester, students requesting absence verification for COVID-19 positive results must provide official medical documentation from a provider noting their positive test results and any additional dates they are to remain out of classes. At-home tests will not be accepted as supporting documentation.

In each case, Student Assistance and Support Services must have documentation in order to verify the circumstance, take action, and advocate on behalf of a student. Our office may take steps to call medical providers or others to validate any documentation submitted. Submission of falsified documentation by a student may result in charges under the Code of Student Responsibility and inability to utilize Absence Verification in the future.(Required)

Your Absence Verification request WILL NOT be processed without original documentation provided to the Office of Student Assistance and Support Services. Supporting documents may be attached below (preferred formats are PDF or JPEG). HEIC images (an option for iPhone photos) are not a supported; please convert/save any HEIC images as a PDF or JPEG before uploading to this request form.
