Academic Integrity Referral


This form is designed for faculty who seek to refer a suspected academic integrity violation to the Office of Student Rights and Responsibilities. Only the faculty member overseeing the academic exercise in question may refer an academic integrity case. Students who wish to report a suspected academic integrity violation should contact the faculty member of the course or academic exercise in which the violation allegedly occurred.

Faculty may use this online form to upload all materials associated with an academic integrity referral including a signed Undergraduate or Graduate Academic Integrity Violation Form indicating the student's decision. The following information should also be attached to this referral:

    • Course Syllabus

 

    • Copies of any relevant communications between the faculty member and the student regarding the incident

 

    • In cases of plagiarism, a copy of the student's paper and the source text(s) should be provided. The student's paper should have alleged or in question passages highlighted.

 

    • In cases of cheating, copies of the assignments/exams should be provided. Each student's assignment/exam should have alleged or in question passages highlighted.




This referral form, and any supplemental materials which contain personally identifiable information, may be reviewed by an eligible student in compliance with the Family Educational Rights and Privacy Act of 1974.
