UCM Online Feedback and Complaint Form
If you encounter a problem on campus, we encourage you to try to resolve the issue informally with the individual or department directly involved. Many issues can be resolved by making an appointment with a faculty or staff member and calmly and honestly communicating the concern.
If you are not satisfied after working informally to resolve the concern, you may wish to file a formal complaint.
Once a formal complaint under this process is received, an attempt will be made to review and resolve issues appropriately and in a timely manner. If you believe it is important that you be contacted directly about a concern or its resolution, please indicate this on the form below.
Several areas within the university have defined processes to respond to student concerns or complaints:
Academic Complaints: When appropriate, a complaint may be referred directly to the department involved. Complaints related to a specific academic college may be forwarded to the appropriate college Dean. All other academic complaints, or if the appropriate academic department can not be determined, should be directed to the Office of the Provost. Please refer to the Academic Appeals Procedure, for a complete explanation of these procedures.
Student Conduct Concern: A description of university student policies and expectations can be found in UCM's Guide to Good Decision-Making.
Criminal Conduct: Anyone aware of a crime can make a report by contacting the Office of Public Safety at 660-543-4123. They can also use these Reporting Forms. Please be aware that all UCM Employee's are required to report certain criminal acts including sexual assault, sexual misconduct, harassment or child abuse or neglect. Additionally, Campus Security authorities are required to use the Mandatory Report form to report crimes. It is located here: Mandatory Report Form. Data collected from criminal reports is used in the development of the UCM's annual Security and Fire Safety Report.
Discrimination/Harassment: The University of Central Missouri actively follows a policy of nondiscrimination in regard to age, race, color, religion, sex, sexual orientation, gender identity or expression, marital status, pregnancy or parental status, national origin, veteran status, genetic information, disability, and all other legally protected classes. This policy applies to educational programs and activities including athletics, instruction, grading, the awarding of student financial aid, recruitment, admission, employment, housing, placement and retention of students, faculty and staff. The university complies with applicable federal and state laws and regulations related to discrimination. If you believe that you have experienced discrimination, you may use this form to make a report. For more information, please review UCM's Discrimination/Harassment Procedures.
Sexual Misconduct: The University of Central Missouri believes that the educational environment should be free from sex discrimination. Sexual harassment of students, including sexual violence and other forms of sexual misconduct, interferes with this right and will not be tolerated. Sex discrimination (including sexual harassment, sexual assault, and sexual violence) is prohibited at UCM. Federal Guidelines have been implemented here at UCM for responding to reports of sex discrimination. These reporting guidelines are in response to the Office of Civil Rights 2011 letter regarding Title IX, the federal law prohibiting sex discrimination in educational institutions. These procedures apply to all employees, students and third parties. Rape and all sexual violence are included as severe forms of sexual harassment under Title IX regulations. This means that all verbal or written reports of sexual assaults that occur on campus, at campus-related events, or that relate to discrimination, harassment, or retaliation alleged on campus, must be investigated by the university. Reports of sexual violence may be made at the Office of Public Safety, the Office of Student Experience and Engagement, University Housing and the Office of Human Resources. Public Safety will investigate all reports and make the appropriate contact with UCM staff. Students who do not want to make a report but would like assistance are encouraged to contact the 24 hour Confidential Hotline at 660-441-4855, UCM's Counseling Center at 660-543-4060, the University Health Center at 660-543-4770 or the Title IX coordinator at 660-543-4114 for assistance in addressing sexual violence or for other Title IX Resources.
Maintenance Request: Maintenance requests, commonly known as work orders, are requests submitted when there is a maintenance issue that requires attention in a residential room, apartment or common living space. Work orders can be submitted 24 hours a day using the Web Work Request online or by calling Facilities, Planning and Operations directly at 660-543-4331. You can also check the status of your work request by calling this number. Another way to submit work orders it to contact the front desk of the building you are in or the CA. The hall staff member will then contact the on-call staff member to respond to the situation. Contacting the front desk or hall staff is the best approach to submit an emergency request after regular business hours.
Student Governance: Students are also encouraged to bring concerns and related issues to the attention of the Student Government Association (SGA) through standing committee structure. SGA meetings are open to the public and many ideas are brought forward in these forums. SGA regularly meets with the Vice Provost for Student Experience and Engagement with the opportunity to bring forth campus-wide student concerns.
