Hazing Incident Reporting Form


This form should be used to report acts of hazing that may violate University policy and/or Oklahoma state law. Please refer to the UCO Code of Conduct for definitions of hazing ( Code of Student Conduct) or find the state statute by clicking this link, Oklahoma statute. Please provide detailed information regarding the incident you are reporting. Do not use this form to report other violations of the UCO Student Code of Conduct.

Background Information

Though not required, we ask that you please provide your name and contact information. Your identity will be held in confidence. It will not be disclosed to the individuals or organization in question. Please report the acts of hazing even if you choose to omit your name and contact information. If you believe a student is in immediate danger, please contact the University of Central Oklahoma Police Department at (405) 974-2345. Thank you.
