General Report


The purpose of this form is to inform the University of Dubuque (UD) of incidents so that UD can follow-up with action when appropriate. Information gathered may also result in an investigation by Human Resources or the Office of Student Life or designee. Statistical information gathered may be reported in UD’s Annual Security and Fire Safety Report per the Jeanne Clery Act.

Reminder: You DO NOT need to know all the answers to the questions below. In addition, in completing this form, you are NOT expected to investigate further or ask individuals specific questions as part of completing the document

If you have personally experienced sexual harassment, sexual assault, dating/domestic violence or stalking, you may want to review information about support resources, protective measures, accommodations, and response options available to you before completing this reporting form.

If you are otherwise reporting in compliance with campus requirements, completing this form allows our campus community to meet the reporting requirements listed below:

  • Campus Security Authorities (CSAs): Designated in compliance with the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (Clery Act).   • Title IX Responsible Employees (REs): Designated in compliance with Title IX. Required to report any information that suggestions a violation of the UD’s Policy on Sexual Harassment and Sexual Violence.   • See Something, Say Something: Assist in keeping the UD Community safe by reporting things you become aware of that may disrupt the community.
  • Bias or Hate Incidents:  If you are in immediate danger, please call 9-1-1.  The primary goal is to provide support to the individual or community impacted. Reports will be evaluated to determine if further investigation is required for potential violations of University policy and/or criminal law.
  • Anonymous Reporting Hotline: The University of Dubuque is committed to providing a safe campus. To assure this the University has a confidential third party reporting service. Its purpose is to provide a convenient tool to anonymously and confidentially report activities that may represent criminal, unethical or otherwise inappropriate behavior in violation of university policies. Anonymous reports may limit the University's ability to respond to the incident.

If you want to report a crime to local law enforcement, please call Dubuque Police Department at 911 or 563-589-4415, or come to UD’s Campus Safety and Security Department located at Smith Hall, Room 112 at 2205 Grace Street or by calling 563-589-3333. UD Campus Safety and Security is open 24 hours a day, every day of the year.
