Academic Early Alert Reporting

Instructors, your early feedback about specific behaviors may signal to students that they need to take corrective actions to their behaviors if they want to succeed academically. Your participation supports our student retention efforts and our desire to help them.

NOTE: We simply don't collect information; we act on the information you provide us (but we do ask for your feedback). The best way we can support students is to share concerns with constructive feedback that encourages positive and improved student behaviors.

We strongly recommend that you speak with the student(s) prior to submitting an Alert. We hope that your conversation with the student will be the first step to improving his/her behavior. If after speaking with the student you notice that s/he has not made improvements then let us know about the demonstrated early signs of distress that might put him/her “at-risk” of withdrawing or failing your course later in the semester.

WE WILL follow-up with the student and campus allies, as needed, about your concerns. When you submit a report, a copy is sent to the Dean of Students, Athletics Department, and other campus allies, as needed. Correspondence is also sent to the student. Thank you!

Please note that information presented within this report will become part of University administrative record and protected by FERPA privacy laws. If you have any questions regarding filing an incident report, please contact the respective office:
