Incident Report Form

This Incident Report Form is available to any person wishing to report a:

a) Suspected student prohibited behavior under the Code of Student Conduct;
b) An incident of Sexual Misconduct as outlined by the ULV's Title IX policy (i.e. sexual assault, relationship violence, stalking, etc.); and/or
c) A disruptive community behavior or incident.

Individuals wishing to report a wellness concern or behavioral concern should use the Behavior & Wellness Referral Report.
Individuals wishing to report an incident of social injustice or discrimination should use the Social Justice Reporting Form.

The Dean of Students Office administers the Code of Conduct and Title IX complaints against students. The Code of Student conduct outlines behavioral expectations for all students. The Title IX policy prohibits gender discrimination in all programs and activities at the University of La Verne. Upon receipt of a report, a University staff member will seek to meet with identified individuals to investigate and address the alleged prohibited conduct.

Reporter Background Information

Reporters are encouraged to provide as much or as little of their information to assist the University with appropriate follow-up to a report. Information submitted in this report will be used by the University for possible action in accordance with the Code of Student Conduct or the University's Title IX policy.

Reporters wishing to make an "anonymous" report may do so by writing "anonymous" in the name field. While anonymous reports will be evaluated in the same manner as "named" reports, the University may be hindered in its ability to follow-up.
