This form is designed for incidents relating to the Office of Residential Life.
This form allows the staff in the Office of Residential Life, residents in the residence halls/on-campus apartments, students, and visitors with an online method to report specific information related to an alleged incident(s) regarding conduct or behavior that may be interrupting the educational environment.
This reporting form is to be used for non-emergency purposes only. If this is an emergency, please call University Police at 318.342.5350 and Residential Life professionals.
