Student Group Conduct Incident Report Form


Complete this form to report a student group that has 1) violated one or more student group policies, 2) violated the Student Conduct Code (including hazing), or 3) violated their own constitution, policies, or procedures. Please attach additional files or articles of evidence that you wish to submit with this form. Reporter Information

Name and contact information of the reporting party is required as the reporting party will be contacted during the review process to learn more about the reported incident or violation. Anonymous reports can be filed through the University's confidential reporting service, EthicsPoint, however, these are not submitted directly to Student Unions & Activities and are not specific to Student Group violations.
