You may use this form to report an incident that happened to you or to another individual. This form is for reports about UMW students.
Reports about faculty/staff should be directed to Human Resources and for incidents involving honor code violations (lying, cheating, stealing), please direct to The Honor Council.
Examples of incidents that could be reported on this form include concern about a student's behavior, a bias incident, sexual and gender-based violence, interpersonal violence, or any other form of emotional or physical violence. Depending on when you submit this form, it might not be reviewed until at least the next business day. Your privacy will be respected, and the information you share with us here will be shared with other individuals only on a need-to-know basis.
Please be aware that all submissions connected to the Policy on Sexual and Gender-Based Harassment and Other Forms of Interpersonal Violence (Prohibited Conduct) reported through this form will be shared with UMW's Title IX Coordinator. Using this form does not constitute a Formal Complaint for the purposes of Title IX and the Prohibited Conduct Policy. Options for responses may be limited if the report is anonymous.
