UNLV Student Conduct Incident Report


The Office of Student Conduct (OSC) at the University of Nevada, Las Vegas administers the Student Conduct Code, which identifies the University expectations for all students on and off campus. If you have knowledge of a UNLV student violating the Code, you are encouraged to notify our office of the incident using this form.

Once a report has been received by OSC, our staff will investigate, determine to meet with involved parties to address the alleged misconduct, and resolve the case per the Student Conduct Code. Please know that you and the witnesses you identify in the report might be contacted by OSC for further information about the incident. Additionally, please note that University conduct records are protected by the Family Educational Rights and Privacy Act (FERPA).

Anyone providing false or inaccurate information may face appropriate conduct and/or legal action at the discretion of the Office of Student Conduct. Please provide a detailed description of the incident using specific, concise, and objective language. The use of direct quotes is encouraged even in incidents when the alleged student uses profanity or abusive language. Include the details of the incident(s) and the behaviors observed. Focus on the Who, What, Where, When, Why, and How. If you have listed an individual as an involved person, they should appear at least once in the narrative. Please refrain from using pronouns (he, she, they, etc.) to refer to people in the body of the report--referring to individuals consistently by last name is preferred.
