Supporting UNG Nighthawks

The information collected through this form is used to better understand the concern(s) you have for yourself or another UNG student. A staff member from the Dean of Students office will reach out to offer support and resources.

BEFORE YOU BEGIN: If this is an emergency that involves an imminent risk of harm to self or others, please call Public Safety at 706-864-1500 or dial 911.

If you are concerned about a student, faculty or staff member who is exhibiting self-destructive or disruptive behaviors, please make a referral using the See Something/Say Something form.

General Information

If you are completing this form to express concern for someone else, we do not require your name to respond to these cases. However, our ability to address anonymous incidents can sometimes be limited. It is often very helpful to know who is making the referral so we can follow-up most appropriately.
