When an undergraduate student takes a medical withdrawal from the University of Richmond, the student is expected to receive appropriate treatment for the problem(s) necessitating the withdrawal during their absence from campus. Before the dean of the student’s respective college may make a decision regarding the appropriateness of readmission, the student must fully complete the questions below to help university staff assess the student’s readiness to return to their academic program and, if applicable, residential living.

Once submitted this form will be sent to your college Dean, CAPS, and the Student Health Center. The information you provide below will be reviewed in conjunction with the "Treatment Summary and Recommendations" from your treating health professional.
