Sex & Gender Discrimination and Sexual Misconduct Incident Report Form

This form is for reporting incidents that fall under USF's Policy on Nondiscrimination Based on Sex and Gender, Sexual Harassment, and Sexual Violence, including sexual assault, dating and domestic violence, and stalking. Submissions using this form may not be reviewed outside of normal business hours.

Reports filed through this form are immediately received through the system and reviewed during same-day business hours, or the next business day, by the following offices: Title IX Office (for all incidents), Vice President of Student Life, Dean of Students, and Human Resources (for employee-related incidents).
