Once the form is complete, click "submit", and you will be contacted by the Dean of Students or a representative from Student Affairs.

If your issue is of an academic nature, your form will be routed to the appropriate faculty member, department or dean via the Office of Academic Affairs and will be handled in accordance with the UST policy for Academic Grievances. Before submitting this official complaint, you should make all attempts to work out your complaint with the faculty or staff involved.
