Study Away Health and Safety Incident Report Form


The University of Wisconsin-Eau Claire takes the health and safety of students studying away from campus on international and domestic programs very seriously. In the event that you or someone you know experiences a health or safety incident while away from campus, we strongly encourage you to report the incident. Most importantly, reporting allows UW-Eau Claire to provide support or resources that might be needed to manage a health or safety concern. Reporting an incident also allows UW-Eau Claire to track concerns at our partner sites. A health concern would be any illness, injury that requires professional medical attention or that resulted in your inability to participate in the program for a day or more. Safety concerns include a variety of issues such as theft, physical assault, sexual harassment, hate crimes, natural disaster, etc. and can be subjective as what makes one person feel unsafe may not impact another person. When in doubt, please report so that the situation can be assessed.

This form is not intended for use in emergency situations. If this is an emergency, please contact local staff at your host institution and/ or local police.
