Hazing Incident Report


Hazing includes acts that endanger the physical health or safety of a student for the purpose of initiation or admission into or affiliation with any organization operating in connection with a school, college, or university—regardless of a person’s willingness to participate. Hazing is incompatible with the standards of the University community.

Examples include, but are not limited to: use of alcohol, servitude, taking away cell phones, not allowing class attendance, requiring apparel that would normally be considered poor taste or humiliating, scavenger hunts for questionable materials, engaging in activities designed to create excessive fatigue, imprisonment, smearing of substances on skin, or forced consumption of food and/or other substances.

Anonymous Reports:

This reporting form allows reporters to submit without including their name, but doing so can significantly limit the University’s investigative processes to hold organizations and/or students accountable for hazing. Without sufficient supporting information, anonymous hazing reports are unlikely to result in a finding of responsibility for hazing.

If you submit anonymously, please share as much detail as you can including:

  • How were you made aware of the possible hazing behaviors? If you heard about possible hazing from someone else, how were they made aware?
  • List the possible hazing behaviors in as much detail as you can.
  • Share dates, times, and locations where possible (even approximations are helpful).
  • If you have documentary evidence (photos, emails, texts, etc.) to support your allegations, please attach them at the end of the reporting form.

Background Information

The University will investigate all reports submitted to the extent possible. So that reports may be thoroughly investigated, we encourage reporting persons to include their names and contact information.
