The purpose of this form is to report Incidents of bias or hate crimes occurring on or off campus to the University's Bias Response Team.
- Bias Incident: Acts of prejudice targeting individual race, ethnicity, age, class, gender, sexual orientation, sexual expression, physical abilities/qualities, religion, nationality, or any other differences that are not crimes and do not involve violence, threats, or property damage.
- Hate Crime: A crime motivated by bias against race, color, religion, national origin, sexual orientation, gender, gender identity, or disability.
The identity of the individuals involved will be kept private to the maximum extent possible under applicable state and federal law. Submitting this form may or may not result in criminal or university action as this depends on the parties involved, the location of the incident, and the type of the incident.
You may fill out as much or as little information as you choose; anonymous reports are accepted (report does not contain any personally identifying information about the victim(s)). Note that not including some information might limit the University's ability to respond appropriately. It is the practice of the UW-Stevens Point Bias Response Team to always contact reporting persons and/or victims of bias incidents.
You may also contact the Office of the Dean of Students (715-346-2611) during normal business hours or University Police (715-346-3456) directly for help (24 hrs). If the incident occurred off campus, you may also contact the City of Stevens Point Police Department (715-346-1500).
