This form should be used to report allegations of Sexual Assault, Dating or Domestic Violence, Stalking, Sexual Harassment, or Sexual Exploitation.

You are not required to complete the entire form in order for the complaint to be submitted, unless you are identified as a "Responsible Employee" by the institution. The institution will use the information provided to review for possible investigation, which may include contacting individuals named in the report. However, if the report does not contain specific information, the institution's investigation and response may be limited.
Once submitted, this report is sent to the UW-Stevens Point Title IX Coordinator (Eric Roesler) and members of the UW-Stevens Point Title IX team. Reports are also reviewed by University Police and Security Services to address immediate potential threats to individuals or campus as a whole. UPSS will not contact named individuals unless there is a potentially dangerous situation and does not retain copies of reports unless otherwise indicated below.
