This form is intended to be used for behavioral and conduct matters along with federal/state reporting requirements of a non-emergency nature. For emergencies, please call 911.

Reports are routed to appropriate University personnel based on the category selected in the "Nature of this report" box below. If unsure of which category to choose under "Nature of this report," please select General Incident and Dean of Students staff will re-route as appropriate.
