Referrals are reviewed during normal business hours and are not monitored after hours, on weekends, or during official University holidays. These referrals are used to initiate collaborative action plans to assist the referred student. Though meetings focus on the concerning behaviors and ways we can help, please be aware that the referred student can request the name of the person who referred them.
While referrals from this form are reviewed by a variety of campus partners working to assist students it is NOT designed for emergency response situations. Emergency Response situations should be reported immediately to campus police. That being said, if the information in the referral is determined to be emergency-related, law enforcement may become involved to check on the immediate well-being of our student.
A note on academic reports: We encourage faculty to submit reports on students who may be struggling academically. As the reports are noted in the system and become a part of the educational record, we ask that you not put multiple students of concern on a single report form. This results in student personal information in other student files (ie, FERPA violation).
